Education for Self-Management

This course consists of four sessions in a group setting. Day and evening classes are available. Your support person is encouraged to attend. Classes are taught by a registered nurse (R.N.) and a registered dietitian (R.D.).

  • Meal planning/food choices
  • Illness/emergencies
  • Complications/risk

  • Foot and skin care

Class Dates and Times:

Day Series – Class Times: 9:00 a.m. – 12:00 p.m.

Note: Class #3 is a 1 hour 1:1 Session with the Registered Nurse and Registered Dietitian (time is undetermined until class #1 or #2)

Cost/ Coverage

  • $1020/person. Physician referral is required. Usually covered by medical insurance or medical coupons.
  • Usually covered partially or fully by Medicare, Medicaid, and other insurances with a physician referral. Check with your insurance.
