Education for Self-Management

This course consists of four sessions in a group setting. Day and evening classes are available. Your support person is encouraged to attend. Classes are taught by a registered nurse (R.N.) and a registered dietitian (R.D.).

  • Travel/vacations/holidays
  • Medications
  • Meal planning/food choices
  • Illness/emergencies
  • Complications/risk
  • Research/resources
  • Foot and skin care

Class Dates and Times:

Day Series – Class Times: 9:30 a.m. – 11:30 a.m.

Note: Class 3 is scheduled from 9:00 a.m. to 12:30 p.m. to accommodate a 1 hour One-on-One Conversation and 1 hour lecture. You will only attend ONE of the two sessions offered for Class 3.

Cost/ Coverage

  • $1020/person. Physician referral is required. Usually covered by medical insurance or medical coupons.
  • Usually covered partially or fully by Medicare, Medicaid, and other insurances with a physician referral. Check with your insurance.
