Non Epic Referral Protocol

Thank you for your referral and allowing us to participate in the care of your patients with osteoporosis and osteopenia.

The UW/NWH Strong Bones Program is committed to empowering your patient with the educational and therapeutic resources that prevent disabling fractures.

Every referral is personally reviewed by an osteoporosis specialist/provider to decide the level of care needed.

In order to ensure accurate and timely contact and scheduling an appointment with your patient, please provide the following information. Please be specific with diagnosis code(s) specificity related to Osteoporosis or Osteopenia to ensure accuracy with authorization of patient referrals. If the patient requires authorization please be sure to initiate authorization and provide authorization details on the faxed referral.

Essential for patient care coordination:

  • patient demographic facesheet
  • copy of insurance card
  • ICD 10 codes related to condition and used for authorization
  • most recent chart note documenting diagnosis and referral
  • current medication list
  • last two DXA reports (If Available)
  • Past 2 years of lab results

Upon review, the Strong Bones Coordinator will contact your patient to schedule their consultation.
