Insurance, Referrals & Authorizations
At Regional Vascular Center, our goal is to provide our patients with timely access to a group of highly specialized surgeons. Many times, referrals to specialty health services require prior authorization. Our team will work collaboratively with your physician’s office and your insurance plan to ensure that all of the requirements for referral authorization are met.
We encourage you to review your coverage policy prior to your visit with us. Each plan is different and it is important to understand the extent of your responsibilities, if you have any co-payment or deductible requirements, and if there are any limitations on how you can access the specialty care services you may sometimes need. If you have any questions, please contact your health plan’s member service department.
Patients in need of immediate hospitalization or inpatient services will be admitted to Northwest Hospital & Medical Center unless requested otherwise.
An advance directive, or living will, is a written document that details your direction and treatment preferences should you be diagnosed to be in a terminal condition or permanent unconscious state. This document can inform your family and loved ones about what treatment you want or don’t want when you are unable to advocate for yourself. Please request a form from a member of our staff or inquire with your treating physician for more information.
