Admissions

Your physician or health care provider will make arrangements for your admission to Northwest Hospital & Medical Center. When you arrive, unless you’re told otherwise, please go directly to the Admitting Department, located in the main hospital lobby.

Our admissions representative will help you complete the forms we need, answer your questions and make sure you’re escorted to the correct hospital unit. You will be asked to sign some papers giving us permission to care for you. Some of them may sound daunting, but please don’t be alarmed. They are perfectly routine and we need to have your approval in case something comes up during your stay. You may be asked to sign documents dealing with:

  • Consent for care or surgery.
  • Release of information to your insurance carrier or other organization (please see Notice of Privacy Practices for more information).
  • Responsibility for your hospital charges.
  • Assignment of insurance benefits.
  • Specific services you'll need for your diagnosis.

Regardless of the reason for your stay at Northwest Hospital, please be sure to bring your insurance cards or forms.

Another way to help us take good care of you while you’re in the hospital is to print out and complete this handy personal medication record with information about your doctors, vaccinations and the medications you take.
