Who Can Refer
Any physician, residential care facility, community agency or mental health professional may refer. Family and self-referrals are also welcome.

How to Refer
Referrals can be arranged directly by calling the following program numbers:

• Inpatient Program referrals for admission can be made 24 hours a day at 206-368-1835.
• Outpatient Program referrals can be made by calling the Community Assessment Coordinator at 206-368-1813.
• Questions or referrals to either program can be made by calling the Intake Liaison Nurse or the Community Assessment Coordinator at
  206-368-1747.
