Northwest Primary Care Fremont Clinic Relocating on April 4

Northwest Primary Care Fremont Clinic has provided care to the Fremont community for over 20 years. We are proud to announce that due to our remarkable growth in the Fremont area, we are moving to a new location on Tuesday, April 4th, 2017.

The new location will continue to provide exceptional primary care, family medicine, chronic disease management, preventive care and screening, with expanded services such as onsite lab and digital X-ray. All of this care will be conveniently located on N. 35th Street in the Burke Building (see photo) with free parking.

Our new address will be: 400 N. 34th Street, Suite 203, Seattle WA 98103. Our phone number will stay the same at 206.545.9300.

From wellness and preventive care to managing acute and chronic issues, our primary care physicians provide a continuum of care tailored to your individual health needs.

Our primary care physicians encourage the active involvement of patients in their own healthcare—giving you the guidance you need to stay healthy by improving your overall health and preventing disease from developing in the first place. We also take the time to get to know you and your family, and educate you on the best health practices. At Northwest Primary Care Fremont Clinic, your good health is our goal.

Our services include:

Proper medical care is essential to the healthy physical and mental development of a child, starting from birth, through childhood and adolescence, and to adulthood. We provide comprehensive care for our youngest patients, and many of them grow up with us. Our services include well child exams, immunizations, school and sports physicals, and coordination of care with other specialists when necessary.

For primary care providers, helping to prevent illness is just as important as treating it. Our practitioners focus on helping you to develop the lifestyle habits that will contribute to a long and healthy life. We recommend all of our patients schedule annual physical exams with their providers. An annual physical gives us the chance to check in with you, examine any changes in symptoms, lifestyle or overall health since your last visit, schedule necessary tests, screenings or immunizations and discuss any other health concerns.

Annual exams typically include a check of vital signs, as well as heart, lung, head and neck, abdominal, neurological and dermatology exams. For women, we also often include a Pap smear and breast and pelvic exam in an annual physical, and for men, a prostate evaluation.

We believe that with proper monitoring and care, patients with chronic health conditions can live long, happy and healthy lives. We help our patients manage a variety of chronic conditions, including high blood pressure, diabetes, cardiac disease, lung disease, asthma and allergies, and more. We provide guidance on improving your overall health, such as diet, medication and pain management. We also frequently coordinate and collaborate with multiple specialists, all of whom work with us to achieve optimal health and functioning for our patients.

Continuity of Care Policy

The physicians of Northwest Primary Care are committed to providing their patients with the best care possible, and feel strongly that continuity of care is integral to the delivery of care. Continuity of care is the process by which the patient and the physician are cooperatively involved in ongoing healthcare management with the goal of high quality, cost-effective medical care.

Billing & Insurance

Northwest Primary Care accepts Medicare and most major insurance plans. Please contact your insurance company to verify benefits prior to your visit, or contact us with any questions. Your insurance may require you to pay a co-payment at the time of your appointment. (This is typically noted on the front or back of your insurance card.) Please bring your insurance card and photo ID to your appointment.
